What it looks like:

• Comprehensive wellness or medical consultation

• Detailed review of your medical history

• Thorough discussion of symptoms, concerns, and personal health goals

• Personalized treatment recommendations tailored to your needs

• Review and interpretation of recent or previous laboratory results

• Clinical evaluation to help identify potential underlying contributors to symptoms

• Dedicated time with your provider to answer questions and provide education on available treatment options. This may include supplements, hormone therapies, peptide therapies and other supportive treatments when clinically appropriate and individualized to your needs.

What it looks like:

All of the benefits included in a Consultation, plus:

• Comprehensive laboratory testing tailored to  your specific symptoms, concerns, and health goals

• Lab orders placed through Quest Diagnostics, with same-day blood draws available in our office for your comfort and convenience

• Follow-up appointment to review your laboratory results with your provider, typically within 7–10 days once all results are received

• Detailed interpretation of your results and personalized recommendations based on the findings

• Clear, actionable guidance regarding treatment options, supplements, hormone therapy, peptide therapies, and other wellness recommendations when clinically appropriate based on your individual lab results and health goals.

$399 plus the cost of laboratory testing

Same-day and next-day sick visit appointments are available for infants, children, and adults in Peoria and surrounding areas. Our providers evaluate and treat a wide range of acute illnesses and non-emergency medical concerns, including cold and flu symptoms, sore throat, strep throat, fever, respiratory infections, sinus infections, urinary tract infections (UTIs), migraines, eye infections, minor injuries, lacerations, and more. Our goal is to provide prompt evaluation and treatment so you can start feeling better as quickly as possible.

What it looks like:

• Focused medical evaluation of your symptoms and concerns

• Diagnosis and personalized treatment recommendations

• In-office medication dispensing and administration when clinically appropriate

• Prescriptions sent directly to your preferred pharmacy, if needed

• Access to in-office treatments such as breathing treatments, IV therapy, and medication injections when medically indicated

• Rapid in-office testing with same-day results available for conditions such as UTIs, strep throat, COVID-19, and Influenza A/B, allowing for prompt treatment when appropriate

• Additional send-out laboratory testing available when clinically indicated for more complex presentations, persistent symptoms, or added peace of mind

Additional fees may apply for in-office procedures, laboratory testing, medications, IV therapy, injections, or other treatments recommended during your visit.

What’s Included:

• Review of current symptoms, progress, and response to treatment

• Review and discussion of recent lab results as they relate to your existing treatment plan

• Minor medication adjustments, dose modifications, or refill-related changes when clinically appropriate

• Follow-up after a recent sick visit, new medication, or treatment adjustment

• Ongoing monitoring of established conditions and therapies

• Small refinements to your current treatment plan based on symptoms, progress, and clinical response

• Time to address focused questions related to your existing care

Office Visits are intended for focused follow-up and ongoing management of an established treatment plan. If you have a new concern, need a more comprehensive evaluation, or would like to discuss new treatment options outside your current plan of care, please schedule a Consultation instead.

• Comprehensive pre-participation physical examination for sports clearance

• Review of medical history, medications, allergies, and prior injuries or surgeries

• Evaluation of cardiovascular, respiratory, musculoskeletal, and neurological health

• Assessment of height, weight, blood pressure, vision, and other vital signs

• Discussion of any current symptoms, health concerns, or conditions that may impact athletic participation

• Injury risk assessment and recommendations for safe participation in sports and physical activities

• Completion and signing of required school, club, or athletic participation forms, when medically appropriate

• Dedicated time with your provider to answer questions and discuss overall health, wellness, and injury prevention strategies
